In order to improve and control quality, the underlying problem must be diagnosed. I loved the "scrapping burnt toast" analogy. I see this all the time in my office. The doctor I work for is always trying to adjust our policies based on the outlier. If one thing goes bad they automatically assume that something needs to change, rather then just dealing with that particular problem, and then going back to the original plan.

Motivation
My lesson in my management class this week was also about motivation. WE discussed several motivational theories.
Thinking about my current job I realized how all of the principles discussed are completely true. The days I love my job the most are the days that we have all different types of patients. I love when we have a rare disorder and have to come up with a new treatment regimen just for them. Whereas the days filled with eczema and acne tend to me long, uneventful, and I find myself looking at the clock a whole lot more.
I also really like the idea of participant managing. I am a big fan of bureaucratic management style. As a manager I love getting input about new policies and new ideas that my employees have come up with. Listening to feedback helps improve morale in our office, and helps everyone feel like they are a part of the bigger picture.
Money is also a huge motivator for me. Once I reach a certain place in my life I dont think it will be as big of a motivator, but as of now it is definitley the top of my list.

Notorious V.A.G.
What i learned:
- Why Stephen (and most guys) don't want to work on labor and delivery.
- Reiterated that Joe is bilingual
- Nurses restrict change by not sticking together, throwing each other under the bus, and eating their young.
- In order to make change, and have more of an influence in the future of healthcare, we need to start sticking up for each other, and banning together to get things done. Nurses make up 80% of healthcare providers, there is absolutely NO reason why we shouldn't also be the most powerful.

Strategic planning
Some how all of our lessons seem to somehow coincide with things going on at work. Today in my managers meeting we were discussing one of our biggest issues, scheduling surgeries. As of now we only schedule surgeries on Wednesday and Friday mornings. Mohs surgery is done in stages, ie, the dr takes a single layer of the lesion at a time, processes it onto a slide, then checks it under a microscope to see if the margins are clear of cancerous cells. He continues to take "layers" until the cancer is out. Anyway, he is only in the room with the patient for maybe 10 minutes at a time. So if we dont have other patients scheduled in between the Dr gets upset because he is "wasting time." Lately we have not been able to fill the schedule with surgeries like we used to. Long story short, the other managers have been trying to just been buttering burnt toast. I came out and said what is the real problem here? We ultimately decided that it was the way we are tracking cancerous biopsies. We came up with a new system, fixed the broken toaster, and now im crossing my fingers that it works!
Change
Changing and implementing new policies and procedures can be difficult and frustrating, but they are sometimes necessary in order to improve efficiency and patient care. This lesson taught me how important it is to introduce change gradually, to set goals, follow through, and involve all parties that will be effected int he decision making process. Sometimes it is better to bring in an outside party to help come up with new ideas. Someone who is not emotionally connected to a situation or organization is able to better identify areas of concern and come up with new ideas and solutions.

I interviewed brother who is a general manager of a Hampton Inn. I was surprised to find out that if I was in his position as GM I would likely do things in the same manner as he does. His evaluations and disciples are very fair, and open. He does not beat around the bush with with employees, they have a mutual respect for each other, and they feel like he is very approachable.

A: I shop around a lot to try and find the most efficient way to do things, or the best deal. It is kind of fun, like a treasure hunt to find how much money you can save.

A: Thats why it is important to have extra money in a reserve. Sometimes no matter how hard you try the numbers just don't add up and you spend more then you anticipated. If you have extra money set aside for that purpose you wont be in a bind when that happens.

A: Yes, we make the same amount of money as we have for awhile, but the cost of living keeps increasing, yet the economy has caused compensation and salaries to not increase to match those cost of living increases. It has forced me to be more strict with how and what we spend our money on.
I think that this was a very beneficial conversation for me. Although it dealt with a family budget, all of the answers can be applied to a work environment. It is especially important to be sensitive to the fact that many companies and people are struggling right now doe the the economy. More people are getting laid off, and less people are getting hired. Making a budget and sticking to it can significantly impact the success of a company and make the difference between closing down and staying open through these hard times.
